Number of ACA-related enrollments in the U.S. 2014-2024

As of 2024, roughly 45 million individuals in the U.S. benefited from the Affordable Care Act and were enrolled in some form of ACA-related health insurance. This figure has increased from 12.6 million in 2014, the year ACA took effect. Since then, there has been an increase in the number of people who have become eligible for free or subsidized health care. Individuals can now enroll in ACA Marketplace and be eligible for premium tax credits, they may have become newly-eligible for Medicaid in states that have expanded Medicaid, or were previously eligible but didn't know or were unable to apply. Moreover, some states have introduced the Basic Health Program to provide continuous coverage for low-income individuals whose income fluctuates above and below Medicaid eligibility. * Marketplace effectuated enrollment figures for 2014 and 2015 are as of 12/31/2014 and 3/31/2015 respectively, versus February coverage as of 3/15 for 2016-2021.
**Medicaid enrollment data, 2014-2020, are from the February monthly enrollment (ever enrolled during the month) for the expansion adult eligibility group, as reported by states through the Medicaid Budget and Expenditure System (MBES). 2021 Medicaid enrollment data are from December 2020 monthly enrollment, as this is the most recent available monthly enrollment count from MBES.
*** BHP programs did not start until 2015. BHP enrollment data are based on average monthly (for Minnesota) or quarterly (for New York) projected enrollment submitted by the states to CMS in advance of the applicable quarter and are rounded to the nearest thousand. BHP enrollment data for 2021 is through May 2021.
